The clinical workflow around an ambient scribe is less complex than most setup documentation suggests. The first few consults take a small amount of additional time to confirm consent and familiarise with the interface, and by the end of the first week the scribe is running unobtrusively in the background. What distinguishes a rollout that delivers the published efficiency gains from one that plateaus early is the work done in the first fortnight around consent, templates, and review habits, rather than anything technical about the product itself.

Before the first consult
Three pieces of preparation are worth completing before the first consult, and none of them require more than about fifteen minutes.
The first is consent. Australian privacy law, TGA guidance on digital scribes, and medical defence organisation position statements all require documented patient consent before an AI scribe is used in a consultation. The practical options are verbal consent at the start of each consult, which Lyrebird timestamps in the consent log, or written consent captured once at patient enrolment, or a combination of the two for different patient cohorts.
Most practices arrive at a combination after the first month.
The second is a starting template. A custom template is not required to begin, and starting with SOAP or an issues-based note is a reasonable default. Most clinicians settle into two or three templates they use for the majority of consults, and these tend to emerge from the first week's use rather than being designed in advance.
The third is the microphone. Built-in laptop microphones are generally adequate for a standard consult room. A separate microphone may produce cleaner transcripts in rooms with hard reflective surfaces or where the clinician speaks softly, though microphone quality is not typically the rate-limiting factor on output quality.
During the consult
The scribe is opened, consent is confirmed, and recording begins. The consult proceeds as it normally would, without any requirement to dictate or repeat patient statements for the scribe's benefit. Both sides of the conversation are captured, and non-clinical content is filtered out at the draft stage.
The single workflow adjustment that tends to produce a noticeable difference in draft quality is speaking the assessment and plan aloud during the consult rather than formulating them internally and typing them afterwards. A plan that is not articulated in the consult cannot be transcribed, and the scribe's output will accurately reflect only what was said. For clinicians accustomed to composing plans silently, this is the one habit worth building deliberately in the first week.

After the consult

Edits are made directly in the scribe interface. Recurring edits are a signal that the template or prompt needs adjustment, not that the scribe is underperforming; the same edit appearing on consecutive notes is the clearest indicator of where template work is needed.

The first two weeks

In the first few days, drafts are typically accurate but generic, because the scribe has not yet seen the clinician's preferred structure or phrasing. Over the following week, as templates are adjusted and a small number of the clinician's own existing notes are uploaded as examples, the drafts begin to read closer to the clinician's own style. Review time drops as the scribe adapts to the clinician's patterns.
Clinicians still making substantial edits after two weeks are usually working with an under-adapted template rather than a limitation of the scribe. Common issues
Several patterns recur across deployments, and the GCHHS evaluation and other published case studies identify them consistently.
Over-reviewing is the most common source of eroded time savings. Scribes are not infallible, but they are reliably accurate enough that word-for-word re-reading transfers the documentation burden back to the clinician. Reviewing for clinical accuracy rather than linguistic perfection preserves the intended efficiency gain.
Unspoken plans produce incomplete notes. The scribe can only transcribe what is said, and clinicians whose plans are formulated silently will see that silence reflected in the draft. This is not a scribe limitation so much as a characteristic of ambient capture.
Template work is under-prioritised. The GCHHS evaluation identified template adaptation as one of the factors that separated clinicians reaching significant time savings from those whose gains plateaued. A small amount of time spent adjusting a template in the first week compounds across the working year.
Use in low-value consults is worth reconsidering. GCHHS data showed smaller effects in very brief consults and procedural work, consistent with scribe output being proportional to the amount of clinical conversation captured.
Scaling across a practice
Rolling an AI scribe out across a multi-clinician practice involves a small number of coordination decisions and a light ongoing quality assurance process. Staged rollout tends to work better than simultaneous rollout, with two or three clinicians trialling first and their feedback informing the wider deployment. Consent wording is worth agreeing at practice level so patients receive consistent information across different GPs. Expectations should be calibrated by baseline. A clinician already completing contemporaneous notes will see smaller time savings than one whose documentation routinely extends into the evening, and note-quality improvements are more pronounced where baseline documentation was sparse.
